Training Cardiac Surgeons: Safety and Requirements.

To analyze whether cardiac surgical residents can perform their first surgeries without compromising patients' safety or outcomes, by comparing their performance and results to those of senior surgeons. All documented CABGs conducted between 2002 and 2020 were included. Surgeries were divided according to the experience level of the main surgeon (defined by the number of CABG conducted by him/her) using the following thresholds: 1000; 150; 80 and 35. This resulted in 5 groups: senior surgeons (the reference group); attending surgeons; fellow surgeons; advanced residents and new residents. Primary endpoint was 30 day mortality. A Roadmap to the Pelvic Autonomic Nerves During Transanal Dissection

Current international guidelines and consensus panels emphasize that nerve sparing is a crucial part of the total mesorectal excision (TME). The transanal approach (taTME) could be advantageous in this regard. However, even with taTME, pelvic autonomic nerve preservation requires perfect knowledge of the topographic and morphological aspects of the anatomy and requires surgeons to acquire specific skills. Unusual surgical topography presents a considerable risk of nerve injury, when attempting a TME with the “bottom-up” approach.

Porcine Model for Internal Mammary Vessels Harvesting

Recipient vessels selection is crucial for successful microsurgical breast reconstruction. The internal mammary vessels (IMVs) have gained great popularity, and they are often selected as first choice recipient vessels. The IMVs can be exposed for microvascular anastomosis through a rib-removing (trans-costal) or a rib-sparing (intercostal) approach.1–3 Because the vessels are located between the rib cage and the parietal pleura, adequate training is needed to avoid and eventually manage bleedings and pleural perforation. This training is better performed in a nonhuman model. Our aim was to investigate the pig as a surgical model for IMVs harvesting to be used in training.
